This paper reviews the role of cannabis in diabetes. Cannabis is by far the most commonly used illicit drug in Britain, though its use may be declining. There are an estimated 50,000–100,000 people with diabetes using cannabis, with an unknown number using the drug for self-medication. The evidence of the effects of cannabis on diabetes is complex, ranging from anecdotal reports of benefits and harms to experimental research on cannabinoids. The endocannabinoid system appears to have a role in the regulation of body weight and food intake, and the development of hyperglycaemia, insulin resistance and dyslipidaemia. In experimental models, the main psychoactive constituent of herbal cannabis, Δ9-tetrahydrocannabinol, has been shown to interfere with both the action of insulin and its release. 

The paper also considers the effects of cannabis on complications of diabetes. Experimental work has suggested a mechanism to reduce neuropathy but the only double-blind clinical trial to date of a cannabis-based drug found no difference in the ability of the cannabis-based product to relieve neuropathic pain when compared with placebo.

 In conclusion, new insights into the role of cannabis and cannabinoids in diabetes are emerging from this developing field of research.

이 논문은 당뇨병에서 칸나비스의 역할을 검토한다. 칸나비스는 영국에서 가장 일반적으로 사용되는 불법 약물이지만 그 사용은 줄어들 수 있다. 칸나비스를 사용하는 당뇨병 환자는 약 50,000 ~ 100,000 명으로 추정되며,자가 치료를 위해 약물을 사용하는 사람은 알려지지 않았다. 칸나비스가 당뇨병에 미치는 영향에 대한 증거는 혜택과 해로움에 대한 일화 보고서에서 칸나비노이드에 대한 실험적 연구에 이르기까지 다양하다. 체내 칸나비노이드 시스템은 체중 및 음식 섭취 조절, 고혈당증, 인슐린 저항성 및 이상 지질 혈증의 발생에 중요한 역할을하는 것으로 보인다. 실험 모델에서 식물칸나비스의 주요 정신 활성 성분 인 Δ9-tetrahydrocannabinol은 인슐린의 작용과 방출을 방해하는 것으로 나타났다. 

이 논문은 또한 당뇨병 합병증에 대한 대마초의 영향을 고려하였다. 실험적 연구는 신경병증을 감소시키는 메커니즘을 제안했지만 대마초 기반 약물에 대한 현재까지 유일한 이중 맹검 임상시험은 위약과 비교할 때 신경병증 성 통증을 완화하는 대마초 기반 제품의 능력에 차이가 없음을 발견했다. 

결론적으로, 당뇨병에서 칸나비스와 칸나비노이드의 역할에 대한 새로운 통찰력이 이와같이 발전하고 있는 연구 분야에서 출현하고 있다.
